To become a new patient, please call our office to schedule an initial appointment. You may be asked to complete a brief pre-appointment screening form to ensure that your concerns can be addressed at the visit.
During your appointment, we will gather information about your medical history and discuss your healthcare needs and recommend a treatment plan.
We ask that you provide at least 24-48 hours notice if you need to cancel or reschedule an appointment. This allows us to offer the appointment slot to another patient in need of care.
If a patient "no shows" for 3 visits without clear explanation, we reserve the right to have the patient discharged and transferred to another practice.
Payments are due at the time of service. We reserve the right to postpone future appointments if outstanding balances from a prior visit have not been received before the next clinical visit.
This is not a traditional concierge practice, but there are elements in this practice that are similar. By not accepting most insurance plans, this practice focuses on quality and accessibility to medical care over quantity. You will have direct access to Dr. Cheng for patient messages and short phone calls that would generally be handled during the business hours of 8am-5pm. More complex matters that cannot be handled with brief messaging and correspondences may necessitate a follow-up visit by physician discretion.
If you have a medical emergency, please call 911 or go to the nearest emergency room. If you have a non-life-threatening medical concern, please call our office and we will do our best to fit you in for an appointment.
